The manuscript was probably in use until the Reformation. The parchment flyleaves are from an 11th-century Bible copied by an English hand. Copied for use at Eton College; the coats of arms of Eton College and of its Provost, Henry Bost (d. 1502), are found in decorated initials in the manuscript. 21 pieces are incomplete, and 29 of the 93 pieces listed in the original index are now missing (works by Baldwyn, Dunstable, Mychelson).

Leather covers are probably from the second half of the 16th century; stamped on the front cover are the Tudor Rose, portcullis, fleur-de-lis, and 'H.R.' (the initials of the London-based binder).

Original description describes 8 endpapers, but only one at front and 3 at back were found in situ. These were photographed as i (front) and ff. 127-8, ii (back). Each endpaper is an opened bifolio of another MS (non musical).
