56 paper folios, 560 x 410. Original first gathering now misbound at beginning of ModD 12. Modern foliation, 1-56, on lower center rectos (one original folio missing between new ff. 13/14, another between new ff. 42/43). Covers date from late 16th or early 17th century. No index. Copied by a single scribe, who also wrote parts of ModD 3, 9, 11, and 12. Inked calligraphic initials.

Folio 48v contains the last complete page in this MS of the Richafort Requiem (Benedictus and start of Agnus Dei). Folios 49r to 51r inclusive contain the Osanna/Benedictus and complete Agnus Dei of Moulu's Missa Paranimphus.
