XVIe s. 120 ff. Papier, 405 x 275 mm. Pagination ancienne discontinue en haut des ff. et foliotation moderne au bas des ff. Les ff. 34v, 35, 39, 72, 75v, 76, 87v, 97, 100v, 101, 115v, 116 sont blancs. Notation mesurée blanche. 11 et 12 portées par p. Initiales ornées.
Ce ms. où l'on discerne 6 mains différentes est constitué de messes, motets, Magnificat, hymnes et chansons copiés sans plan défini. Josquin l'emporte avec 7 pièces. Léon X étant désigné comme «pontifex maximus» au no 19 (alors que la même chanson est attribuée au «Cardinal di Medici» dans I-Fn 107bis), le ms. n'a pu être copié qu'après 1513 et avant 1521. L'état présent du ms. reconstitué à partir de feuillets disparates semble être dû à Orazio Vecchi, maître de chapelle à Modène en 1583, car on lit à la couv.: «Jacobi et Ludovici Foliani olim cathedralis Mutinae magistri opera ab injuria temporum a Vecchio vindicata.» (Roncaglia).
