The manuscript has been restored, but the discantus book is incomplete and a few folios are missing from the other books. Pencil foliation; also a stamped foliation and a new pencil corrected foliation. Rebound in new covers. Copied by several scribes. Some red and green initials and rubrics. Inscriptions in discantus book: "Krakowski Jacobus Surnese" and "Has partes emi 1 fiorino ab haeredibus petri stöckel."
After 1558 (MurTVB); date 1558 in manuscript. From the Church of St. Aegidi in Bártfa ( = Bartfeld = Bardejov). Brought to the National Library in Budapest in 1915.
