Original covers of parchment over paste-board. Original index in T book. Probably copied partly by Simon Lyra, cantor at Church of St. Elizabeth in Breslau (Lyra's name on flyleaf). Black or red inked initials in plain block style. Watermarks: (1) resembles Briquet #2263; (2) resembles Briquet #12298 (LassoW).
1593 (date in manuscript). Copied in Breslau, for use in local evangelical services, probably at Church of St. Elizabeth. Later transferred to Breslau Stadtbibliothek. Missing since World War II; contents and composers known from BohH.
