RISM B/IV 4: 14. Jh.; 69 Blätter Pergament, 207 x 150 mm. Foliierung des 18. Jh. von f. 1-69. - Einband: moderner Pergamentband. Die aus Mainz stammende Handschrift des späten 14. Jh. enthält Agenda bona secundum ordinem Moguntinum (vor allem Prozessions-Gesänge) mit gotischer Choralnotation. Das einzige mehrstimmige Stück ist ein Nachtrag: in Partitur auf 2 Fünfliniensysteme. Es ist aber nur die erste Textzeile mit Noten versehen; die übrigen 3 Doppelsysteme sind leer und zeigen nur den Text.
