125 paper folios of an original 155, 410 x 290. The manuscript is in poor condition; ff. 73-102 are missing. Ink foliation, 1-155. Modern parchment covers from ca. 1957, when the manuscript was restored in Rome. No index. Black inked calligraphic primary initials. The last two pieces, on ff. 154'-155', are later additions, probably from the end of the 16th century.
Mid-16th century. Copied and used at San Petronio in Bologna.

Composers: Change "Phinot-12" to "Phinot-13"; change "anon-6" to "anon-5."
