RISM BIV/4: 15. Jh.; 96 Blätter Pergament, 385 x 280 mm. Moderne Foliierung von f. 1-96. Je Seite meist 7 Fünfliniensysteme mit gotischer Choralnotation, vereinzelt mit schwarzer Mensuralnotation (Credo). - Einband: 15. Jh.; Holzdeckel mit Beschlägen.

Diese zwischen 1442 und 1468 geschriebene Handschrift ist ein Spezialgraduale aus der Breslauer Hauptkirche St. Elisabeth. Sie enthält neben zahlreichen Ordinariumssätzen ein Commune Officium de B. Virgine, ein St. Anna-Offizium und Proprien für einige Hauptfeste.
