RISM BIV/3: 16. Jh.; 445 Blätter Pergament, 615 x 410 mm. Moderne Foliierung von f. 1-445; originale Foliierung: a I-a XX, b I-XX..., z I—XI (es fehlen mindestens 6 Blätter). Je Seite bis zu 10 rote Fünfliniensysteme mit böhmischer Choralnotation (zum Teil mit mensuralen Elementen). Reichlich mit Miniaturen geschmückt. — Ein-band: 16. Jh.; mit Pergament überzogene Holzdeckel.
Die auch als Caslauer Graduale bekannte Handschrift ist auf f. 1v datiert: „Erb Města Cžaslawie 1557". Geschrieben wurde sie von J. Táborský, der als Komponist und als Schreiber zahlreicher tschechischer Handschriften bekannt ist. Mehrstimmig sind nur 3 Versschlüsse eines Christe.
Inhalt: Alle Texte sind tschech.; f. 2-54v Kyrie und Gloria, meist tropiert; f. 55/56v Te Deum; f. 57-76v Sanctus, Agnus; f. 77-276v Proprium; f. 277-419v Sequenzen; f. 420-443V Patrem; f. 444 Wappen Táborskýs.
