18 fragmentary paper folios, now of irregular shapes and widely varying sizes (average ca. 245 x 175-200) + 17 smaller scraps and strips, all recovered from bindings. Apparently the only surviving remnants of a choirbook of unknown original size. All fragments mutilated and in very poor condition; some music trimmed away or illegible. Copied by a single scribe. Inked Gothic initials.
Late 15th or early 16th century. Perhaps copied in Lwów. Owned in 20th century by the musicologist Adolf Chybiński (1880-1952); deposited in present library following Chybiński's death.
