Je Seite 10 Fünfliniensysteme mit gotischer Choralnotation, zum Teil mit mensuralen Elementen; p. 352-354 mit weißer Mensuralnotation - Einband: datiert 1526; mit Pergament überzogene Holzdeckel.
Diese im frühen 16. Jh. von Lybomyensky Jakub geschriebene und vermutlich aus der Benediktiner-Abtei in Tyniec stammende Handschrift (mit Nachträgen von 1596) enthält Officia de Beata Maria Virgine per totum annum (p. 1-40 Kyriale, p. 41ff. Officia mit zahlreichen Sequenzen).
