XVe/XVIe s. 119ff. Papier (f. 1 parchemin), 450 x 335 mm. Foliotation moderne en haut des ff. 1, 2, 47v, 48, 119v sont blancs. Notation mesurée blanche. 11 portées par p. Capitales ornées de sujets pittoresques en couleurs. Rehure ancienne cuir brun, très fatiguée.
Au f. 2v, les armes des Maffei, célèbre famille de Vérone, rattachent indiscutablement le ms. à cette ville. Les miniatures, de style un peu amateur, trahissent aussi une origine locale (la ville représentée serait Vérone) ainsi que le papier où l'on retrouve le filigrane de l'oie (semblable à celui du ms. précédent 757) dont la diffusion n'a pas dépassé le nord de l'Italie, spécialement Vérone et Vicence. La copie a dû s'effectuer dans les toutes premières années du XVIe s., sinon dans les dernières du XVe. A une première section, constituée en majeure partie de motets (nos 1 à 27) succède un répertoire d'hymnes et Magnificat destiné aux Vêpres (nos 28 à 58). Les nos 26, 42, 47, 49, 50 ont été copiés postérieurement à la première rédaction du ms.
