This manuscript comes from Poland or Silesia. It contains chants as well as some histories; the only clearly polyphonic music is on f. 56v/57r. The manuscript was in the Biblioteka Zaluskich in Warsaw before 1795; it then came to St. Petersburg, from where it was returned to Warsaw in 1921.

RISM B/IV 4: Seit 1944 verschollen; erhalten sind nur die Photographien von 13 Seiten.
Diese Handschrift des 14. Jh. stammt aus Polen oder Schlesien. Sie enthielt chorale Gesänge in gotischer Notation sowie einige Historien. Auf f. 56v/57r finden sich die einzigen heute noch greifbaren, mehrstimmigen, in schwarzer Mensuralnotation auf 4 Doppelsystemen notierten Stücke. Die Handschrift befand sich vor 1795 in der Biblioteka Zaluskich in Warschau, gelangte dann nach St. Petersburg, von wo sie 1921 wieder nach Warschau gebracht worden ist.
