2 paper partbooks, each consisting of the prints RISM VI115, K989, and D3522, with manuscript additions (B-62 folios, Q-70), 147 x 200. Modern pencil foliation in each book. Original ink numbering of pieces in two series, 1-9 (6-voice works) + 1-26 (5-voice works); some pieces not numbered. Original covers of brown leather stamped with voice designations and date 1579. No index. Copied by one main scribe, with slightly later additions by a different hand. Staff height variable (13-16). Small plain inked initials. Two watermarks generally resemble Briquet #1489 and #1420; two others described in PerzRP.
Last quarter of 16th century (date 1579 on covers; date 1587 in section of later additions. Copied in Olkusz, for use at Church of St. Andrew (inscription at beginning of print section: "Pro usu Ecclesiae Ilcusiensis Sancii Andreae …")
