ii + 22 + ii paper folios, 216 x 165. Modern foliation, 1-22. Modern covers. Plainsong treatise, with musical examples in Gothic chant notation, forms main body of manuscript. Polyphonic piece copied on f. 1, in white mensural notation.

Probably late 16th century (date 1611 on f. 22, at end of section of later additions). Presumably copied in Poland.
