332 paper folios, 305 x 200. New pencil foliation. Covers of brown tooled leather on boards, with metal corners. White mensural and chant notation. Two main scribes; two additional later scribes (1708, 1710). Red rubrics; red, black, green, and gold initials. The manuscript is a Czech Gradual containing monophonic music as well as the polyphonic pieces.

Early 17th century (with 18th-century additions). Copied for the Church of the Assumption of the Blessed Virgin at Neděliště (northwest of Hradec Králové).
