RISM BIV/3: 16. Jh.; 298 Blätter Papier, 390 x 255 mm. Moderne Foliierung von f. 1-298; originale Foliierung von f. 1-100 (= moderne f. 7-91). Je Seite 7 rote Fünfliniensysteme (f. 7-199) mit böhmischer Choralnotation, schwarzer und (in Nachträgen) vereinzelt weißer Mensuralnotation - Einband: 16. Jh.; mit weißem Leder überzogene Holzdeckel, Blindpressung, Beschläge, Schließen.
Dieses im 16. Jh. geschriebene böhm.-utraquistische Graduale stammt aus der Dekanatskirche St. Martin in Sedlcany. Wie eine Notiz besagt, wurde es im Jahre 1593 „erneuert". Den ältesten Teil bilden f. 7-199. Nachträge wurden bis ins 19. Jh. eingefügt.
