Je Seite 8 Fünfliniensysteme mit böhmischer Choralnotation (zum Teil mit mensuralen Elementen). - Einband: 16. Jh.; mit weißem Leder überzogene Holzdeckel, Blindpressung (Datierung des Einbandes: 1563).

Diese Handschrift bildet den 4. Band des großen utraquistischen Gradual-Kantionals der Stephanskirche der Prager Neustadt, geschrieben in der 2. Hälfte des 16. Jh. (im 4. Band Jahreszahlen: 1585, 1591, 1594). Das einzige, in weißer Mensuralnotation geschriebene, mehrstimmiges Stück ist ein Nachtrag und steht auf einem eingeschobenen Blatt (originale f. V 19bis).
