RISM BIV/3: 16. Jh.; 364 Blätter Papier + 1 Nachstoßblatt, 310 x 210 mm. Moderne Foliierung von f. 1-364; originale Foliierung von f. A I-XX bis S XVI (originale A I = moderne f. 7). Je Seite 7 rote Fünfliniensysteme mit böhmischer Choralnotation und schwarzer Mensuralnotation - Einband: datiert 1563; mit dunkelbraunem Leder überzogene Holzdeckel.
Dieses utraquistische Graduale wurde um die Mitte des 16. Jh. für den Literatenchor von Žiželice (Bezirk Hradec Králové) geschrieben. Später gelangte der Band in die Bibliothek des theologischen Instituts von Hradec Králové.
