RISM BIV/3: 16. Jh.; 574 Blätter Papier + je 1 Vorsatz- und Nachstoßblatt, 292 x 200 mm. Moderne Foliierung von f. 1-574; originale Foliierung von A I-XX bis Ee XI (= moderne f. 1-564); Pag. 1-53. Je Seite 5 rote Fünfliniensysteme mit böhmischer Choralnotation, schwarzer und weißer Mensuralnotation - Einband: 16./17. Jh.; mit weißem Pergament überzogene Holzdeckel, Messingbeschläge und -schließen.
Dieses utraquistische Graduale und Teilantiphonar wurde im Jahre 1597 vom Literatenbruder Joh. Hradecký aus Hradec Králové für die Marienkirche von Lochenice (bei Hradec Králové) gestiftet. Geschrieben ist es 1586. 1705 wurde der Codex zensuriert und einige Stücke ausgestrichen.
