Je Seite 9 rote Fünfliniensysteme mit böhmischer Choralnotation (zum Teil mit mensuralen Elementen), schwarzer und vereinzelt weißer Mensuralnotation — Einband: 1584; mit Schweinsleder überzogene Holzdeckel, gut erhaltene Beschläge und Schließen (restauriert 1838).

Die mit prächtigen Miniaturen und ganzseitigen Illuminierungen ausgestattete Handschrift ist bekannt als Graduale (1. Teil) des Matous Radous (Illuminator und Maler aus Chrudim).Sie wurde, wie auch der 2.Teil (CS-HKII A 13b), zwischen 1585 und 1604 für die utraquistische Literatenbruderschaft der Heiliggeist-Kirche von Hradec Králové geschrieben. Die Nachträge reichen zum Teil bis ins 18. Jh. hinein.
