RISM BIV/3: 16. Jh.; 431 Blätter Papier + 2 Blätter Pergament, 530 x 360 mm. Böhmischer Choralnotation und weiße Mensuralnotation - Einband: 16. Jh. (Werkstatt J. Komor, Prag); mit weißem Leder überzogene Holzdeckel, Blindpressung.
Dieses utraquistische Kantionale, das um 1572 geschrieben worden ist, konnte wegen seines sehr schlechten Zustandes nicht inventarisiert werden. Die Handschrift befand sich 1965 in der Konservationswerkstatt der tschechische Staats- und Universitätsbibliothek in Prag. - Mehrstimmiges findet sich ab f. 241v (in weißer Mensuralnotation).
