RISM B IV/3: 16. Jh.; 207 Blätter Pergament, 585 x 385 mm. Moderne Foliierung von f. 1-207. Je Seite 8 rote Vier- oder Fünfliniensysteme mit böhmischer Choralnotation, schwarzer und (für die Nachträge) weißer Mensuralnotation -Einband: datiert 1559; mit weißem Leder überzogene Holzdeckel, Blindpressung, Beschläge und Schließen.
Dieses um die Mitte des 16. Jh. geschriebene, böhmische-utraquistische Gradual-Kantionale stammt aus der Dekanatskirche St. Martin in Sedlčany. Heute fehlen zahlreiche Blätter (1651 waren es noch 244 Blätter). 1720 wurde die Handschrift zensuriert, wobei 13 Blätter herausgeschnitten worden sind.
Inhalt: Die Texte sind teilweise lat., teilweise tschech. - f. 18ff. Kyrie, Gloria; f. 33ff. Graduale, Alleluja; f. 70ff. Patrem; f. 107ff. Sanctus, Agnus; f. 112ff Kantionen, Motetten; ab f. 203v Nachträge.
