RISM B IV/3: 16. Jh.; 486 Blätter Papier, 470 x 310 mm. Originale Foliierung (zum Teil falsch) von f. I-III, anschließend A I-XX... bis W IV; darnach folgt alte Pagination von p. 1-106 (mehrere Blätter fehlen). Je Seite meist 7 (letzte Seiten 8) rote Fünfliniensysteme mit böhmischer Choralnotation (zum Teil mit mensuralen Elementen; ab p. 56 schwarze und vereinzelt weiße Mensuralnotation — Einband: 16. Jh.; mit Leder überzogene Holzdeckel, Messingbeschläge und Schließen.
Die heute in Rychnow n. Kn. aufbewahrte Handschrift ist ein tschechisch-utraquistisches Graduale aus der zweiten Hälfte des 16. Jh., das aus der Kirche von Kostelec nad Orlici stammt.
